To receive our HIPAA Business Associate Agreement please send us an e-mail requesting same to HIPAACompliance@ReliableHealth.com. Be sure to identify the name of your facility. Within two to three days a customized agreement will be sent to you via e-mail. After you receive the agreement print two copies, have them signed and return both copies to:
